PARAMEDIC? Back in the early 80's a new phenomenon began in Calif. It was the addition of adding Paramedics to Engine companies. The idea was increase the level of service to ALS on Engine & Truck companies, while reducing the response time. Not every department has their own ALS EMS service, thus this addition to pre-existing companies saved money and time. Smaller version of Waterous Midship with Hercules CAFS. The old version was two studs longer. Removed was 1 cross lay, and 3 1x4 bricks.
One issue about 75' HDL's is you can only store so much equipment.. In this case it was a mix of needs v. wants.
On the Fall 2012 models, LDH will be stored above in a 3 1x2x2 box ahead of the turntable.
EMS Equipment and a Blower.
I'm rather thrilled to say this truck carries a full functioning ground ladder. When it was first drafted it wasn't an option.

The bigger tires of you LDD tiller look better. Go with those.
Also, you need more ladder storage.

But I commend you for having real internal ladder storage. Hardly anyone does that so I am impressed with your work.
